Scoresheet

The Volleyball Rally Scoring Scoresheet is an 82k word document and will open in Microsoft Word.

Libero Tracking Sheet

The MHSAA Libero Tracking Sheet is an 14k .pdf document and will open in Adobe Acrobat.

High School LineUp Card

The MHSAA LineUp Card should be used by all coaches to report their starting lineups. Serving position one (1) should always list the FIRST server, not the starting positions on the floor. It is an 22k .pdf document and will open in Adobe Acrobat.

Medical ID Card

Medical ID cards are being used by CAAVO in cooperation with the MHSAA. We feel this will assist in keeping our officials as safe as possible if an emergency should arise while performing their officiating duties. This card will print DUPLEX on a full sheet of paper for you to cut to a size that works for you. It may work best if you print it out on cardstock paper.

If you have never filled out one of the emergency medical contact forms, please take the time to do that so we can have a record or who your emergency contact would be if something should happen to you while you are officiating (this includes before and after your matches while you are on the event premises). If you have already filled out one of the emergency contact forms in the past, but feel you need to update some of your information, this is the time to do that as well.

CAAVO Post Season Online Availability Form

Each year, CAAVO assists the MHSAA Tournament Selection Committee by recommending members in good standing for post season tournament assignments. These assignments include MHSAA Districts, Regional, State Quarter Finals, Semi-Finals and Finals. While CAAVO only directly assigns District assignments, we do recommend our members to the MHSAA Selection Committee for the Regionals, Quarter Finals, Semi Finals and Finals. These recommendations are based on your MHSAA Varsity Ratings and your contributions to CAAVO and the sport of volleyball.

To assist you in being considered, CAAVO has created an online form which will streamline the process and make it painless for you to submit your information, availability, and desire to work post season matches. It is important to fill this form out as accurately and completely as possible. Inaccurate entries such as incorrect Varsity Rating for last year or the last time you worked a MHSAA State Tournament Match could lead to your name being bumped from the list for Upper Lever Post Season Tournament Assignments.

This form must be submitted prior to September each year to be considered for post season assignments.

The form can still be submitted after September if any of the information you submitted changes or to be considered for other late post season assignment requests for officials or line judges.
